Transcript of Pathways to Community Living Initiative - Western Sydney Local Health District

Louisa Albornoz: I’d like to acknowledge the traditional owners of the land on which we meet today – which is the Dharug people – and pay my respects to elders past and present.

The Pathways to Community Living Initiative is a program that’s run across NSW Health, which looks at assisting people that have had very long stays in hospital to transition back out into the community.

Milu Francis: Since the commencement of this program, we’ve supported and transitioned over 110 patients back into the community with a combined length of stay of 580 years.

We also noticed a significant reduction in re-admission rates.​
