Transcript of Mona Vale Hospital Patient Led Handover Project

Vanessa Thomas: I’d like to acknowledge the traditional owners of the land – the Gammeraigal, Kuring-Gai and Dharug peoples of the Eora Nation and pay my respects to elders past and present.

So the aim of our project was to engage our patients in the handover and we did that by implementing a patient-led handover where we went in and asked the patients about their day, what they’ve been up to, and any issues that they had.

The outcomes of the project were that patients were better able to express their needs. We were better able to understand how they felt about their therapy and how they were going.

​ We were also able to feed back to the doctors and therapists about ongoing needs for the patients.​​
