Local health districts and speciality networks manage their own information, as well as information pertaining to the hospitals and public health services captured by their geographical boundaries.

Informal requests, or formal applications under the GIPA Act for information of this nature should be directed to the relevant local health district or speciality network GIPA unit.

Health information and medical records are generally held by the hospital or health facility where you received care.

To make a request for your health information or medical records, please contact the relevant hospital's general number and ask for their medical records department. Their officers will be able to assist you with the process for applying for access to your records.
