Disclosure log

A disclosure log is a record containing details of government information already released under the Government Information (Public Access) Act 2009 (GIPA Act) where a decision was made that the information would be of interest and benefit to the general public.  If a decision has been made that the information released would be of interest to the wider population, the information released becomes part of the disclosure information as open access information. Once information is considered open access information an agency is required to make the information available, if requested, pursuant to Section 25 of the GIPA Act.

Requests for personal information are not included in the disclosure log. Some information that has been released under the GIPA Act is provided in summary format only in this disclosure log. If the information is not provided electronically below, interested parties may access copies of information released under the GIPA Act by contacting the NSW Ministry of Health by email or by post at Locked Mail Bag 2030, St Leonards NSW 1590, Australia.
