The Ministry of Health has transitioned the Integrated Care, Collaborative Commissioning and Leading Better Value Care programs to be managed by a single team, the Connected Care and Partnerships Team, in its System Sustainability and Performance Division.

In March 2023 the Ministry of Health transitioned the Integrated Care, Collaborative Commissioning and Leading Better Value Care programs to be managed by a single team, the Connected Care and Partnerships Team, in the System Sustainability and Performance Division. 

Value based healthcare will continue to be led by the Ministry’s Strategic Reform and Planning Branch. It will have a greater focus on helping overcome system barriers to embedding value based healthcare, while existing programs are integrated into business as usual.

Value based healthcare is transitioning into a new phase

Value based healthcare remains a strategic priority and continues to be an important approach to help us achieve the Future Health strategy objectives. 

Previously, value based healthcare focussed on specific programs to advance our understanding of key areas of value and these programs remain important. Now our approach to value has matured, we are moving our focus towards embedding value more broadly in business as usual. We will do this by addressing the strategic and structural enablers of value, including measuring outcomes and commissioning for value. 

Alignment of the three priority programs will support greater consistency

The Ministry’s Connected Care and Partnerships Team will support streamlined implementation, communications and governance for districts and networks, reducing duplication and increasing consistency. It will assist NSW Health to work together with its partners including the primary care sector to deliver more seamless care for the community.

To reflect the realigned governance a Connected Care in the Community Leadership Group is being established with representatives from each local health district, specialty health network and primary health network, including Ministry of Health representatives. This group will replace the previous Integrated Care Leadership Group and Leading Better Value Care Executive Sponsors Group.

The Connected Care and Partnerships Leadership Group will drive value in line with the Quadruple Aim: through improving patient and population experience and outcomes, provider experience, and efficiency and effectiveness for service delivery.

It will apply a value based healthcare lens and focus on improving the coordination and provision of care in the community and inter-relationships between settings. 

Shireen Martin, Director of the Connected Care and Partnerships portfolio, will be the system manager leading operational implementation and taking a whole patient journey approach to improving patient and population outcomes.
