The Single Digital Patient Record will transform how people experience and deliver healthcare across NSW. It will do this by providing a full picture of a patient’s story in one place, regardless of where individuals and families go to seek care within NSW Health services.
All clinicians need the right information, at the right time so they can provide the best care possible, especially in emergencies.
Right now, a person’s health journey, including hospital visits, stays, treatment, past and recent care, might be split across different hospitals or clinics. The Single Digital Patient Record will bring it all together into one secure digital record. So, no matter which NSW Health service is visited, the healthcare team will have access to the right information when it is needed most.
The Single Digital Patient Record will provide a statewide integration of electronic medical records (eMR), laboratory information management system (LIMS) and patient administration system (PAS).
NSW Health currently uses 9 different eMRs, 10 PAS and 5 LIMS to manage patient records, administration and laboratory information. This means patients often have to repeat themselves when visiting health services and staff don’t always have the full picture.

A new online patient portal called MyChart is being explored as part of the rollout of the Single Digital Patient Record.
Some of the potential functions patients may be able to access via MyChart include upcoming appointments, medications, a range of results, as well as discharge and care plans.
This online portal will be piloted across the phased rollout of the Single Digital Patient Record to ensure it meets strict key NSW Health safety, privacy, legal, policy and operational requirements.

NSW Health understands the benefits and importance of continuity of care for consumers in NSW.
A new secure web-based portal, called the Provider Portal that allows specific and approved clinicians, with the written consent of patients, to view some information within the Single Digital Patient Record, is being explored as part of the program’s rollout.
The Provider Portal would support the continuity of safe, quality patient care as patients move through their NSW Health journey to primary care and other health services.
Before NSW Health considers the introduction of the Provider Portal, there are strict requirements, including consent, legal and privacy, technical, training and support, and other important factors that need to be addressed.
An initial pilot will be undertaken to identify use cases for the Provider Portal and how the key requirements would be addressed.
The rollout of the Single Digital Patient Record will occur in 5 stages. The first go-live includes Hunter New England Local Health District, Justice Health and Forensic Mental Health Network and NSW Health Pathology (Hunter New England sites) in March 2026.
The Single Digital Patient Record will be operational across NSW Health LHDs and SHNs by the end of 2028. Ongoing readiness and testing of the Single Digital Patient Record are currently underway and will include activities such as training staff on how to use the new system, closer to each stage of delivery.
The delivery of the Single Digital Patient Record is focused on a whole-of-health, collaborative approach. Clinicians, patients, carers, consumers, and technical experts are involved and helping shape how and what it will look and feel like. This is important for making sure it meets the needs of those who use it and supports the delivery of high-quality care.
NSW Health is not building the Single Digital Patient Record technology itself. Instead, it has engaged Epic Systems, one of the world’s leading electronic medical record providers, to supply the core technology platform.
The Single Digital Patient Record Implementation Authority (SDPRIA) leads over 57 working groups and councils with members from across NSW Health, including local health districts, specialty networks, and pillar organisations who meet regularly to make sure the program aligns with how care is provided in NSW.
Amazon Web Services (AWS) is providing the secure cloud environment to host the Single Digital Patient Record, and RLDatix Galen is providing the statewide data archive.
