The Quick Access Response Team (QuART) Model of Care describes an allied health–led transdisciplinary model that delivers rapid assessment, intervention, and coordinated support for patients who are medically stable but require short-term allied health input. The model enables timely, home-based intervention to support early discharge and hospital avoidance. The model was originally developed, implemented and evaluated by Illawarra Shoalhaven Local Health District (ISLHD).
QuART strengthens system performance by enabling earlier allied health involvement in the patient journey, improving care coordination across settings, and supporting clinicians to work collaboratively across traditional discipline boundaries.
