QuART Implementation Toolkit

​Model of Care

The Quick Access Response Team (QuART) Model of Care describes an allied health–led transdisciplinary model that delivers rapid assessment, intervention, and coordinated support for patients who are medically stable but require short-term allied health input. The model enables timely, home-based intervention to support early discharge and hospital avoidance. The model was originally developed, implemented and evaluated by Illawarra Shoalhaven Local Health District (ISLHD).

QuART strengthens system performance by enabling earlier allied health involvement in the patient journey, improving care coordination across settings, and supporting clinicians to work collaboratively across traditional discipline boundaries. Evidence from implemented services demonstrates that QuART:

• reduces avoidable hospital admissions and length of stay
• supports earlier discharge and improved patient flow
• reduces hospital bed day utilisation and associated costs
• lowers incidence and risk of hospital-acquired complications, particularly for older patients
• improves patient experience through care delivered in the home
• optimises allied health workforce capability through transdisciplinary practice
• enhances coordination across hospital, community, and primary care services.

Implementation Toolkit

To support statewide scaling and operationalisation, an Implementation Toolkit has been developed for QuART. The Toolkit provides guidance for LHDs/SHNs seeking to implement the QuART model, while maintaining fidelity to the evidence-based service.​​
