Partnered Pharmacist Medication Charting (PPMC)

​​Summary

Partnered Pharmacist Medication Charting (PPMC) is a collaborative model of care that enables a credentialled pharmacist to chart medications in accordance with a Medical Officer's agreed medication plan. By embedding pharmacists and medical officers in a structured partnership, PPMC supports safe, accurate and timely medication charting while optimising prescribing decisions and strengthening multidisciplinary care.

PPMC has demonstrated positive outcomes for patient safety in NSW and is an established model of care in several other Australian jurisdictions. Evidence from existing trials shows that PPMC can:

• Reduce medication errors compared with standard charting practice
• Improve medication reconciliation and support compliance with the National Safety and Quality Health Service (NSQHS) Medication Safety Standard
• Reduce hospital length of stay
• Generate healthcare cost savings
• Support pharmacists and medical officers to work collaboratively at their optimal scope of practice.
